A guest on a package ordered breakfast — will they be charged twice?

No. Tell the till which room you are serving and an included item rings up at zero, taking one off what remains. Order more than remains and only the extra is charged.

On the POS screen, use the guest button at the top to say which room you are serving. The header then shows what their package still covers — "Breakfast 3 left, Dinner 4 left".

At the till

Order the breakfast and the line comes up at zero, marked Package. The kitchen still cooks it and the stock still moves; only the money is zero.

If they order more than they are owed

The bill splits: the covered part free, the rest charged, on two lines. Two free and two paid on one bill, which is what the guest needs to see.

Room service too

A request taken at the front desk works the same way — the same balance, whether the guest walks to the restaurant or rings down for it.
